• Change Personal Information

    Address Change-Employee shall notify location administration and Human Resources regarding a change in address/name by clicking on Address Change.
    -Benefits Coordinator will assist employee in completion of appropriate forms. 

    Direct Deposit Change Form
    -Complete form and submit to Business Office

    Name Change
    -Employee shall contact Human Resources to change a name. 
    -Appropriate documents will be requested to provide adequate proof of name change, such as updated driver's license and social security card, marriage certificate, divorce decree or court order.
    -Benefits Coordinator will assist employee in completion of appropriate forms. 
    -Employee shall change name and email address through the K12 Data Center portal.

    Beneficiary Change
    -All beneficiary changes must be made with the Benefits Coordinator. Contact Human Resources for guidance on updating the appropriate forms.
